Send Photos
Please provide clear, recent close-up full-face photos portrait, profile, oblique, and nasal base. Photos should clearly show the area of concern (nose) and can be sent by mail or e-mailed (preferred). This is especially important for patient who have had previous rhinoplasty and or have a cosmetic deformity.

Obtain an Evaluation
Past medical records such as a history, physical exam findings, photographs, prior operative reports, or imaging can provide important context to your care. If you have these records, please provide them as soon as you can. It is ideal to have access to them prior to the consultation, but Dr. Hamilton will also review them afterwards if there is a delay. It is always best to obtain an in person evaluation from Dr. Hamilton. Dr. Hamilton needs to visualize your perforation in order to provide you with surgical options. If you cannot obtain an in person nasal endoscopy in the office, we will ask you to get a CT scan locally or take a video with a small camera at home. Instructions will be provided for any option you choose. Whether your consultation is in person or virtual, you and Dr. Hamilton will come up with a customized treatment plan together. At the end of the consultation or soon after, our surgical coordinator will reach out to discuss preoperative care, costs, recovery, and any questions you may have.

Read, Initial, Sign
Shortly after your surgery is scheduled, you will receive a DocuSign in your email to sign your financial agreements. You will also be instructed to complete your preoperative forms in our patient portal. All documents must be read carefully and all instructions followed closely. If you do not understand the content of the documents, please do not sign them.
Surgery Disclosure Policy
Dr. Hamilton is a double board certified physician who will perform your entire surgery from start to finish. Although we have engaged in research studies and contributed to many scientific journals, we are not a surgical training center. We do not allow residents, fellows, or other trainees to perform any portion of your surgery.

Please allow ample time approximately 2-4 weeks prior to your procedure for paperwork to be processed by the U.S. State Department and your government consulate. Recovery-AFTER
Repair OF SEPTAL PERFORATION
We advise all patients that fly-in for surgery to spend at least the first night after their procedure in a local hotel or after-care facility, thus ensuring convenience for any required medical attention. After repair of your septal perforation we will see you post-operatively and, depending upon the procedures that have been performed, you may return home within after 5-7 days after arrival in Los Angeles. In the event that you require any medical attention after returning home, we will refer you to a local physician for a check-up. Your health and safety remain our number one priority.
